Avoiding the “R” word in Healthcare: Readmissions

Since 2012, health systems across the United States have been challenged with penalties designed to bring better patient care through value-based initiatives. A large part of that conversation involves a focus on reducing readmissions, especially among Medicare-eligible patients. Penalties, in the form of reduced Medicare reimbursements, impact hospitals of all sizes by hundreds of millions of dollars every year. And unfortunately, the shortfalls can often have a direct negative impact on
