Team-based care is not new to healthcare professionals, but many do not realize the many benefits of that approach. Team-based care can involve many professionals, but a team-based approach should include at least two caregivers. While the biggest benefit of team-based care is patient satisfaction, additional benefits include comprehensive treatment, care coordination, efficiency and effectiveness.
Perhaps moreso than any other process, team-based care is highly patient-centric. A larger provider team might drive higher quality improvement, but also requires effective communication and coordination. Team-based care can also drive continuous quality improvement within healthcare organizations.
For healthcare professionals, one of the key benefits is better time management and a more manageable workload, which typically results in higher patient satisfaction from
greater focus and a simplified work schedule.
Adopting a Team-Based Care Approach (AHRQ):
Engage patients in setting practice-level procedures and policies.
Enlist a core group of patients to provide ongoing input and contribute to planning processes.Offer opportunities for all patients to provide input.
Develop, communicate, and use the practice’s philosophy of patient-centered team-based care.
Achieve buy-in from leadership.Communicate the philosophy widely.Incorporate the philosophy into human resources policies and procedures.
Hire team members who match the practice’s philosophy and prioritize patient-centered care.Hire provider team members who come from and understand the communities served.
Prepare provider teams to apply the practice’s philosophy of patient-centered team-based care in clinical encounters.Train team members to engage patients as partners in care, build relationships, and invite and respond to the needs and preferences patients express.
Introduce patients to the concept of team-based care, while continually seeking feedback.
And providing care extends beyond the hospital and office visits. Regular callbacks with patients after discharge are very important to ensure the patient is following recommended procedures and recovering as planned.
