Platform Delivers Improved Coordination of Patient Care and Monitoring of Provider Metrics
PREMEDEX is pleased to announce that Senior Independence has selected the industry-leading PREMEDEX technology platform to enable their network with greater visibility into all patients, providers and participants.
Senior Independence is partnering with a leading Ohio accountable care organization to coordinate care and improve quality and patient satisfaction through a program called Home to Stay.
Home to Stay helps patients with complex care needs make successful transitions from hospital to home. After conducting a comprehensive review of their needs and available solutions, Senior Independence felt that the PREMEDEX solution provides the most complete set of capabilities that is also highly efficient for implementation and management of patient records and provider information.
“We selected PREMEDEX as they understand the nuances in the world of accountable care organizations,” says Joyce Miller Evans, vice president and chief information officer for Ohio Presbyterian Retirement Services, Senior Independence’s parent organization. “They have built a system that works within many aspects of the transition of care. The unique ability to provide key metrics from the start was a significant criterion for our decision.”
“Senior Independence is an innovative leader in addressing both patient care and business challenges. We are equally excited at our selection by Senior Independence, as this partnership will provide a strong foundation for addressing the needs both of patients and the ACO as they navigate today’s healthcare landscape,” commented Van Willis, President of PREMEDEX.
